Sensitive Teeth

How brief dentine sensitivity differs from decay, cracks and pulp pain—and which home or professional treatments address the actual cause.

Clinical review required: Brief sensitivity can come from exposed dentine, but decay, cracks, pulpal inflammation, leaking restorations and recent treatment can feel similar. Spontaneous or lingering pain, swelling, fever, trauma, pain that wakes you, or rapidly worsening symptoms needs prompt assessment.

What are sensitive teeth?

Dentine hypersensitivity is a short, sharp pain from exposed dentine in response to cold, touch, air, sweetness or other stimuli that cannot be explained by another dental disease. This definition matters: sensitivity is diagnosed after decay, fracture, pulp disease and restoration problems are considered. Not every cold-sensitive tooth has the same treatment.

How dentine becomes sensitive

Enamel normally covers the crown and gum tissue protects much of the root. Recession, periodontal treatment, tooth wear or loss of a cervical restoration can expose dentine. Open microscopic tubules transmit fluid movement toward the pulp, triggering nerve response. Treatments generally reduce nerve excitability or block these tubules.

Common symptoms

Dentine hypersensitivity is usually brief and linked closely to a stimulus. Pain often stops soon after cold air, water or touch is removed. Lingering thermal pain, spontaneous throbbing, pain on biting or one tooth becoming progressively worse suggests another diagnosis and should not be managed indefinitely with sensitivity toothpaste.

Gum recession

Recession exposes root dentine and may follow thin tissue anatomy, inflammation, periodontitis, tooth position, brushing trauma or restoration contours. Treating sensitivity does not move the gum back. The clinician also assesses whether the recession is stable and whether root-coverage surgery, cervical restoration or monitoring is appropriate.

Acid erosion and tooth wear

Frequent dietary acid, reflux, vomiting and some habits can soften and remove tooth surface. Brushing immediately after an acid challenge may increase wear. Management addresses acid frequency and medical causes while protecting teeth. Sensitivity products alone cannot replace lost structure or control ongoing erosion.

After whitening

Temporary sensitivity is common during or after peroxide whitening and often resolves when exposure is reduced or paused. Concentration, contact time and individual pulp response matter. Severe or persistent pain requires review for decay, cracks, recession or an unsuitable protocol. Applying excessive product or extending wear time can worsen symptoms and irritate gums.

After fillings, crowns or cleaning

Teeth may react temporarily after preparation, bonding, cementation or periodontal cleaning. A high bite, exposed root, deep cavity, pulpal inflammation or bonding issue can prolong symptoms. The expected duration depends on the procedure and diagnosis. Increasing pain or inability to bite should be reviewed rather than simply waited out.

Decay, cracks and pulp pain

Caries can expose or irritate dentine; cracks can cause sharp cold or release pain; irreversible pulpitis may produce lingering or spontaneous pain. Visual examination, bite tests, pulp sensibility tests and radiographs help differentiate them. A desensitising paste can mask symptoms temporarily but cannot repair decay or a structural crack.

How sensitivity is assessed

The clinician maps teeth and triggers, then examines recession, wear, restorations and plaque. Controlled air, cold or tactile tests may reproduce the familiar pain while comparison teeth provide context. Radiographs are selected when decay, pulp or apical disease is suspected. A diagnosis should be recorded before irreversible treatment.

Desensitising toothpaste

Formulations may contain potassium salts, stannous fluoride, arginine, calcium sodium phosphosilicate or other agents. Evidence supports several categories, but products and study quality vary. Use consistently as directed; benefit may take repeated applications. Spitting without vigorous rinsing can retain active ingredients, depending on manufacturer instructions.

How to use toothpaste effectively

Brush gently twice daily with a soft brush. A small amount can sometimes be applied directly to a sensitive site according to product instructions. Switching products every few days makes response difficult to judge. Highly abrasive whitening pastes may worsen exposed-root wear, so abrasivity and goals should be considered.

Fluoride and professional agents

Fluoride varnish, high-fluoride prescription toothpaste where indicated, oxalates, resin sealers and other in-office products may reduce symptoms. No single option works for everyone. Professional treatment should follow cause control and be reviewed; repeated coating without addressing erosion or recession may provide only temporary relief.

Bonding and cervical restorations

A bonding resin or composite can cover an exposed, worn or decayed cervical area. Restoration is more appropriate when structure is lost, a defect traps plaque or sensitivity persists. Overcontoured fillings can inflame the gum, and preparation may remove more tooth, so a visible recession alone is not an automatic reason to fill.

Gum grafting

Root-coverage surgery may improve tissue thickness, aesthetics and sensitivity in selected recession defects. Complete coverage and complete sensitivity relief are not guaranteed. Interproximal support, tooth position, defect size, smoking, hygiene and graft technique influence predictability.

Laser treatment

Various lasers are marketed for hypersensitivity. Reviews show heterogeneous protocols and comparisons, and durable superiority over topical agents is uncertain. Ask what mechanism, evidence, risks and follow-up apply to the proposed device. Laser treatment does not diagnose or repair a crack or inflamed pulp.

Diet and daily habits

Reduce the frequency of acidic drinks and foods, avoid holding them around teeth and use water with meals. If acid exposure occurred, rinse with water and allow saliva time before brushing. Do not scrub sensitive roots or use abrasive powders. Dry mouth and reflux symptoms may need medical coordination.

When root-canal treatment is appropriate

Root-canal treatment is not a routine treatment for dentine hypersensitivity. It is indicated when the pulp is irreversibly inflamed or infected and the tooth is restorable. Persistent sensitivity requires a revised diagnosis before endodontic treatment, especially when several teeth respond similarly.

Preventing recurrence

Control erosion, use an effective gentle cleaning technique, maintain fluoride exposure and treat active gum disease. Monitor recession and tooth wear with photographs or scans when needed. A protective appliance may reduce mechanical wear in selected patients but will not control dietary or gastric acid.

Treatment abroad and records

Before veneers, crowns or whitening, document sensitivity, pulp tests, recession, cracks and existing restorations. Ask whether symptoms are expected to change after treatment and who will assess persistent pain after travel. Cosmetic coverage should not substitute for diagnosis.
